Medi-Cal CalAIM (California Advancing and Innovating Medi-Cal) is a comprehensive initiative aimed at improving the quality of life and health outcomes for Medi-Cal enrollees by addressing their physical, behavioral, and social needs. Vivant Health Community Programs works with our Medi-Cal contracted health plan partners to enhance the CalAIM program at the local physician office level by integrating a range of supportive services.
Our skilled navigators, social workers, and nurses help facilitate member participation in CalAIM programs. These may include enhanced care coordination, community health worker engagement, and access to social services that address determinants of health, such as housing, food security, and transportation.
By providing these additional services, Vivant Health Community Programs can ensure a more holistic and person-centered approach to healthcare, thereby improving patient outcomes and satisfaction.
