The Evolution of Vivant Health

Formation and Early Years (1990s)
In the early 1990s, California aimed to transition Medi-Cal beneficiaries into managed care programs to address rising healthcare costs and limited access to primary care. In 1992, community physicians founded River City Medical Group (RCMG) to better serve their Sacramento-area Medi-Cal patients. RCMG initially operated under a primary care case management (PCCM) contract with the California Department of Health Services, managing and coordinating healthcare services for Medi-Cal beneficiaries. -
Expansion Through Health Plan Partnerships (1995–1998)
By 1995, RCMG had partnered with early Medi-Cal Managed Care health plans as part of the Sacramento Geographic Managed Care program. This partnership enabled RCMG to offer capitated primary care agreements, significantly growing its membership and establishing RCMG as the largest primary care organization for Medi-Cal in Sacramento County. In 1998, with expanded health plan involvement, RCMG was tasked to improve culturally appropriate access to primary care, patient health education, translation services, and transportation assistance. This period coincided with a statewide expansion of Medi-Cal Managed Care, further enhancing RCMG’s reach and impact. -
Expansion of Partnerships and Services (2008–2010)
In 2008, RCMG began partnering with additional health plans for Medi-Cal Managed Care, marking the start of a significant service expansion. By 2010, RCMG had adapted to a major expansion of Medi-Cal benefits, including services for seniors and individuals with disabilities, leading to the development of a broader network of internal medicine and specialty services. -
Strategic Growth and Diversification (2012–2024)
In 2012, RCMG's board initiated a strategic shift toward community health and patient are and the development of innovative health service solutions. This led to partnerships with community organizations and hospitals, new programs in transitional care, and social services integration. By 2015, RCMG began contributing to policy development and also entered into partnership with California Northstate University School of Medicine in the creation of a Faculty Practice Plan. In 2016, it acquired the Greater Sacramento Surgical Center. The organization entered the Medicare Advantage program in 2018, and in 2021, it launched its first Medicare ACO program. From 2022 through 2024, RCMG developed a multitude of programs and services to better serve its members, spanning population health management, pulmonary medicine, critical care, physical therapy, women’s health, sleep medicine, and healthcare technology. -
Unifying Our Suite of Programs and Services Under One Brand–Vivant Health (2024)
In September 2024, RCMG rebranded as Vivant Health, marking a new era in its journey and symbolizing its evolution from a conventional IPA to an innovative, quality-focused, population health-centric, technology-driven suite of programs and services now unified under the name Vivant Health. This rebranding reflects a renewed commitment to innovation, quality care, and sustainability. Vivant Health now serves over 270,000 members, focusing on delivering high-quality care through data-driven metrics and advanced technology solutions.
We embrace an “all roads lead to quality” philosophy, ensuring our services are accessible, affordable, effective, and patient-centered. This commitment supports whole-person care and addresses social determinants of health for both Medi-Cal and Medicare populations. For example, our community health programs address food insecurity and housing stability.
We strive for excellence in healthcare delivery, clinical outcomes, and patient experience, continuously improving quality and efficiency through innovative practices. For example, our AI-driven tools and quality management hub support proactive care and early intervention, enhancing both patient outcomes and operational efficiency.
We act with honesty, fairness, and transparency, fostering a positive and inclusive environment. This aligns with our goals of whole-person care, quality population health management, and reducing health inequities across Medi-Cal and Medicare Advantage programs. We ensure that all interactions, whether with patients, providers, or staff, are conducted with the utmost respect and dignity.
We advance healthcare through cutting-edge technology and innovation, developing new care pathways that enhance patient outcomes and promote healthier lifestyles. Our AI-driven tools, such as Intelligent Virtual Agents and predictive analytics, streamline patient interactions and improve operational efficiency. Additionally, our Quality Management Hub leverages advanced monitoring and analytics to maintain high standards of care and compliance. Our telehealth platform expands access to care for rural and underserved communities, significantly improving health outcomes. These innovations support improved care coordination and health outcomes for both Medi-Cal and Medicare Advantage beneficiaries.
