Population Health Management is highly valued due to its strategic use of data analytics to identify and address health trends within the population. This approach emphasizes preventive care and the effective management of chronic conditions, leading to numerous benefits. It offers insights into patient populations, enabling more informed decision-making and personalized care plans. Members experience enhanced health outcomes through targeted interventions and continuous monitoring, ensuring they receive timely and appropriate care, ultimately leading to a healthier, more informed population.
A dedicated provider services team at Vivant Health offers comprehensive support in care management, handling billing, and claims to reduce administrative burdens. This team also educates providers on new opportunities and industry developments, ensuring they are well informed and equipped to deliver quality care. By resolving issues and streamlining processes, the team allows providers to focus more on patient care rather than administrative tasks.
Quality Improvement Programs aim to enhance clinical outcomes and patient satisfaction through systematic, evidence-based efforts. By regularly monitoring and reporting on performance metrics, these programs provide insights into healthcare delivery, identifying areas for improvement and evaluating the effectiveness of changes. For providers, this feedback guides clinical practice enhancements and fosters a culture of excellence. Through data-driven strategies, these programs ensure all stakeholders work towards the common goal of delivering exceptional healthcare.
Patient Education and Engagement programs provide resources that promote better health management and encourage active participation in healthcare decisions. By equipping patients with knowledge about their conditions and treatments, these programs empower them to take control of their health, resulting in more informed and compliant patients, improved clinical outcomes, and more efficient care delivery. Patients gain confidence and satisfaction from being actively involved in their healthcare, leading to better health outcomes and a more collaborative and proactive healthcare environment.
A broad network of primary care providers, specialists, and telehealth providers enhances access to care, ensuring timely and appropriate treatment and supporting comprehensive care management for better patient outcomes. This diverse network improves patient satisfaction and engagement by providing convenient and seamless healthcare experiences. Overall, such a network offers flexibility and convenience, making it highly valued by providers, patients, and health plans alike.
Specialty Care Coordination is a core function of an IPA that facilitates referrals and ensures timely access to necessary specialty care. This service streamlines the process of referring patients to specialists, ensuring that patients receive the specialized care they need promptly. This coordination reduces the administrative burden on providers and enhances their ability to manage patient care effectively.
Coordination with Integrated Behavioral Health Services incorporates mental health services into the overall care plan, addressing patients’ holistic needs. This approach enhances the management of both physical and mental health, leading to improved overall health outcomes. By providing comprehensive care that includes coordinating members into behavioral health, providers can better support patients, and patients benefit from a more thorough, supportive, and effective healthcare experience.
Vivant Health’s professional staff are experts in Medi-Cal and Medicare programs, offering invaluable support to network IPA providers in compliance and regulatory requirements. Providers receive assistance in meeting state and federal healthcare regulations, ensuring they remain compliant with Medi-Cal and Medicare standards. This service is beneficial as it helps providers navigate complex regulatory landscapes, reducing the risk of non-compliance penalties and enhancing the quality of care they deliver. By ensuring adherence to these standards, Vivant Health enables providers to focus more on patient care and less on administrative burdens, ultimately fostering a more efficient and effective healthcare environment.
Vivant Health’s Enhanced Care Management Programs focus on high-risk and complex patients, offering intensive case management that leads to improved health outcomes and reduced hospital readmissions. Vivant Health’s ECM team has years of experience in managing challenging cases, resulting in more effective patient care. Their work has resulted in decreased hospital readmissions and a more comprehensive, personalized care plan that helps patients manage their conditions more effectively.
Vivant Health’s community programs team collaborates closely with community-based organizations to create health program partnerships that address social determinants of health. By incorporating these programs into members’ care plans, Vivant Health enhances the overall health and wellness of the community. This program is highly beneficial for patients as it provides comprehensive support that extends beyond traditional healthcare. Addressing factors such as housing, nutrition, and social support improves patients’ overall well-being and leads to better health outcomes. These partnerships ensure that patients receive holistic care tailored to their unique needs, promoting a healthier, more resilient community.
Vivant Health understands the benefits of implementing value-based care initiatives that align provider incentives with quality and efficiency goals. These initiatives encourage cost-effective care without compromising on quality, which is highly beneficial to providers. At Vivant Health, formerly River City Medical Group, our Utilization Management decisions are based on a careful review of cases by medical professionals. All medical decisions are made using evidence-based guidelines, ensuring the highest standard of care. These guidelines are available upon request by calling (916) 228-4300. Each case is reviewed for appropriateness of care, verification of coverage, and the allowable levels of benefits in the plan. Vivant Health does not reward practitioners or other individuals for issuing denials of coverage. Additionally, we do not offer financial incentives to utilization management decision-makers to encourage decisions that would result in under-utilization of care.
